Youth Leadership and Development Training – Wisconsin

The WI Statewide Transition Initiative receives guidance from a Youth Advisory Board (YAB). The materials contained here were developed to teach workers to help support meaningful work by the Board via youth leadership training and development. The YAB description provides the details and goals of the program. The facilitators guide and presentation structured the training, and the youth activities and programming resource support implementation.

Partnership Educational Presentation and Handouts: Oklahoma

As part of the Oklahoma Real Choice Systems Change Grant, Oklahoma developed a number of training tools for their partnership members. The group consisted of Consumer Consultants and Grant Partners. The presentations and handouts describe roles, working as a team, communication, decision making, policy history, state specific divisions and services and the ADvantage program.

People Talking to People: Yearly Reports

People Talking to People Building Quality and Making Change Happen accomplishes not just a system change, but also a paradigm shift. Their goal is to create a system of quality assurance and quality improvement that has consumers providing feedback. The feedback gathered during the three year period allows for system wide quality improvement by giving consumers and their family members a voice, thus able to control important choices and decisions in their own lives.

Being an Effective and Influential Guardian in Changing Times

Subtitled \"An Information Booklet for Guardians of Individuals who are Protectively Placed and Living at a State Center\", this booklet provides guardians with an updated view of changes currently taking place within the state of Wisconsin. It also encourage guardians to take a more active role in the planning and review processes that determine the appropriate residential setting for the persons they represent and encourages guardians to contact the Guardian Mentor program with questions.

Assessing Utah\'s Long-term Care System

These reports are reviews of Utah’s long–term care system and waiver programs and provide recommendations for change. The Focus Group Findings focused on non-institutionalized and community based settings. The Long-Term Care paper provides more generalized findings about the structure of Utah’s long-term care system and how to transition forward based on input from stakeholders and administrators, comparisons to other states, and review of current documents and policy.

The Guide: NASDDDS Handbook on Inclusive Meetings and Presentations

These guidelines were written to ensure that people with disabilities receive the support and assistance they need to fully contribute to NASDDDS sponsored activities, however the issues, challenges, and solutions offered are universally applicable to the involvement of all self advocates. This document provides a framework for involving recipients of state-funded developmental disabilities services in meetings and activities in an effort to obtain their valuable expertise perspective.

A Case for Inclusion - An Analysis of Medicaid and Americans with Mental Retardation and Developmental Disabilities

United Cerebral Palsy conducted a holistic analysis to chart each state’s ranking and progress in creating a quality, meaningful and community-inclusive life for those Americans with mental retardation and developmental disabilities served by that state’s Medicaid program. This is part of a three-phase process that also will study top-performing states and then create state-specific action plans on how their state can improve.

Report on Colorado\'s Rural Seed Grant Program, 2004-2005

The Dept. of Health Care Policy & Financing awarded four grants totaling $125,000 to improve services to rural residents with disabilities in the areas of non-emergent medical and specialty transportation, adult day and mental health services. The programs are designed to remove transportation barriers, to increase the availability of psychiatry and to provide adult day services. This report summarizes program outcomes, analyzes the opportunities and challenges and outlines the lessons learned.
