Center for Transition to Adult Health Care for Youth with Disabilities Launches New Website

The Center for Transition to Adult Health Care for Youth with Disabilities, a national resource center funded by ACL’s Administration on Disabilities (AoD), recently launched a new website to help young people with intellectual and developmental disabilities (I/DD) direct their own transition to the adult health care system. Developed for and by youth with I/DD, the site has a variety of both person-centered and culturally competent tools and resources to help support youth, their families, and health care professionals through this transition. CMS Launches Webpage to Share Innovative State Actions to Expand Medicaid Home and Community-based Services

On Thursday, October 21, 2021 CMS launched a new “one-stop shop” for state Medicaid agencies and stakeholders on Medicaid.gov to advance transparency and innovation for home and community-based services. Through this new webpage, state Medicaid agencies and stakeholders can access information about states’ plans to enhance, expand, and strengthen home and community-based services across the country using new Medicaid funding made available by the American Rescue Plan Act of 2021 (ARP).

ACL Announces $1.4B in ARP-funded Grants to Kick-off Older Americans Month

ACL issued grants totaling $1.4B to help older adults recover from the COVID-19 pandemic, funded through the American Rescue Plan. These grants will help provide meals and nutrition services, support family caregivers, help older adults connect and engage with others to reduce social isolation, re-open senior centers, and help residents of nursing homes resolve complaints.

Annual Report on Centers for Independent Living - Program Year 2019

ACL recently released the Annual Report on Centers for Independent Living for program year (PY) 2019. CILs offer support for individuals with disabilities who want to live independently in their communities. The CIL program provides financial assistance to local CILs and other CBOs. This annual report includes data from CIL annual program performance reports, federal program staff oversight and compliance activities and publicly available information on grantee websites between 9/30/18 - 9/29/19

Short-Term Money Follows the Person Extensions Resulted in a Significant Drop in State Efforts to Transition People Out of Institutions

The Community Living Policy Center (CLPC) released a brief describing how short-term extensions of the Money Follows the Person (MFP) program have curtailed state efforts in transitioning individuals out of institutions. The CLPC based their findings on CMS data on annual transitions in each state.

HCBS Conference 2019- Presentations

The Home and Community Based Services (HCBS) Conference is hosted annually by ADvancing States (formerly known as NASUAD). This event highlights best practices from across the country in home and community-based services. Promoting Information Sharing by Dual Eligible Special Needs Plans to Improve Care Transitions: State Options and Considerations

In 2021, under a recently released rule from CMS, D-SNPs will be required to notify the state or state’s designee when enrollees experience Medicare-covered hospital or skilled nursing facility admissions in order to ensure timely transitions of care. The Integrated Care Resource Center has released a brief that examines approaches from three states – Oregon, Pennsylvania, and Tennessee – to develop and implement information-sharing processes for their D-SNPs that support care transitions.

Bridging the Health and Housing Gap Transitioning Medicaid Recipients from Institutions to the Community in the Context of Housing Shortages and Affordability

This report reviews federal health and housing policies, noting the barriers created that seniors with clinically-driven housing insecurity must navigate. It also reviews a series of initiatives that ACAP members are undertaking to address housing as a social determinant for this vulnerable population, with examples from California, Ohio, and North Carolina.
