Participant Handbook for IRIS - Wisconsin’s Self-Directed Program

This handbook will help participants in the IRIS Program organize and catalog the information they need to self-direct. The handbook includes information on resources, support and services plan, arranging for services, supports and goods, a glossary and FAQ section. The handbook can be downloaded in English (Large Print and Audio), Spanish, Russian, Hmong, and ASL (movie).

Work Activity and Use of Employment Supports Under the Original Ticket to Work Regulations – Process Evaluation of the Work Incentives Planning and Assistance Program (WIPA)

WIPA was created to give beneficiaries guidance and information on available work incentives so as to increase employment. This report, the first in a series that make up the fifth Ticket to Work evaluation report, has three objectives: to capture stakeholder experiences with the program during start-up; to identify early opportunities for improvement; and to inform future program data collection, evaluation, and outcomes analyses.

School-to-Career Transition Self-Paced Modules

Transitioning from school to work for youth with disabilities can be a complicated process. Sponsored by TransCen and the Johnson Scholarship Foundation, these online modules are designed to provide an overview for these young adults in transition, with a focus on work-based experiences, empowerment, health and social service linkage, family supports and interagency collaboration.

Maryland Bridge Subsidy Demonstration Program

The Demonstration aims to provide 75 to 100 low-income residents with rental assistance for up to 3 years as they wait for permanent housing assistance. A process map showing how a subsidy is provided, marketing materials, sample contracts, and several worksheets for gathering unit and participant information and determining assistance eligibility and amount are given here.

Consumer Handbooks – Minnesota

Here are resources used to educate consumers about MN’s Consumer Directed Community Supports, Personal Care Assistance, and Road to Independence programs. The first handbook gives guidance on the processes and protections of consumer direction and also provides sample care plans and assessments. The handbook on personal care assistance is available in English, Somali and Vietnamese and reviews eligibility, management, and use of services. The last guide assists in moving out of an institution.

Tools for Consumer Participation in Quality Assurance - Florida

Florida created tools to gather information essential to measuring quality from consumers in programs serving people with developmental disabilities and their families. The “Road Map” summarizes the quality assurance process and informs people of their rights. “My Personal Compass” assists in evaluating services received. The last link connects to other resources from the Florida Statewide Quality Assurance Program.

Next Step in Care: Transition Guides and Checklists for Family Caregivers & Professionals

Help family caregivers better manage transitions in care with these guides. Available in English and Spanish, they help determine what issues to consider, how to seek assistance, how to manage medications, and how to better communicate with physicians and other health care professionals. The site, developed by United Hospital Fund, has resources for professionals as well.

It’s Your Choice: Hiring and Managing Your Own Care - Arkansas

Use these training materials to help people with disabilities become skilled employers. The booklet provides specific information about consumer-directed options in Arkansas. The curriculum presents workshops that introduce the consumer direction model and develop abilities related to finding, hiring and supervising employees. Notes for the trainer and handouts are provided for each module.
