Innovative Technology Solutions for Social Care Referrals

On March 4, 2020, the Administration for Community Living (ACL) hosted a National Summit on Health Care and Social Service Integration. At this summit, ACL announced a prize challenge to address Innovative Technology Solutions for Social Care Referrals. The challenge recognizes that states and community agencies have invested in resource directory and referral systems to improve access to long term services and supports (LTSS) for older adults, individuals with disabilities, and caregivers. At the same time, health care providers and health plans may be adopting referral platforms independently to address social determinants of health (SDoH). The challenge suggests a need for greater interoperability and scalability across social service and health care sectors.

Through the challenge, ACL is seeking interoperable referral and analytics technology solutions developed by multi-stakeholder teams that: more efficiently share standardized data to support referral processes and better informed decision-making; incorporate closed-loop referral and follow-up protocols; produce data on referrals, the prevalence of SDoH, and service utilization and outcomes; and develop prototypes and implement technical solutions that track referral patterns and gaps in service that visibly display social service and health related outcomes over time.

The Challenge will compete in three phases with cash prizes awarded in each phase. The total prize award available is $500,000. Preference will be given to multi-stakeholder teams that  include state leaders across aging, disability and Medicaid programs (or their designees), community based organizations in the aging and disability networks, health IT developers, health care providers, health plans, and others with expertise in technical standards, interoperability and data analysis.
