CMS GUIDE Model Participants Announced

The Centers for Medicare & Medicaid Services (CMS) launched Guiding an Improved Dementia Experience (GUIDE) Model, with almost 400 participating organizations building Dementia Care Programs (DCPs) that serve Medicare beneficiaries nationwide. Through the GUIDE Model, CMS will test an alternative payment for participants who deliver support services to people with dementia, including comprehensive, person-centered assessments and care plans, care coordination, and 24/7 access to a support line. People with dementia and their caregivers will have access to a care navigator who will assist in gaining access to services and supports through community-based organizations.

No Wrong Door: Person- and Family-Centered Practices in Long-Term Services and Supports

This paper provides concrete examples of how seven No Wrong Door Systems are promoting person- and family-centered practice. This resource includes a toolkit of resources and contacts for states to learn more and replicate these practices. A checklist also provides a roadmap for states to ensure that No Wrong Door Systems operate in a person- and family-centered way.

Workforce Development and Training Opportunities for Direct-Care Workers

Several of ACA’s provisions directly impact eldercare and disability employers and direct-care staff. This is the first in a series of fact sheets describing these. Key health reform provisions related to investments in direct-care worker training and workforce development are highlighted. These include national advisory panels as well as grant opportunities for states and individual employers. These provisions will be enacted over several years.
