This paper provides concrete examples of how seven No Wrong Door Systems are promoting person- and family-centered practice. This resource includes a toolkit of resources and contacts for states to learn more and replicate these practices. A checklist also provides a roadmap for states to ensure that No Wrong Door Systems operate in a person- and family-centered way.

This resource guide seeks to address some of the challenges of incorporating food security screening and referrals in primary care settings serving older adult patients.Intended for use by health care systems, clinics, and accountable care organizations, the guide synthesizes findings from case studies conducted with health systems that have incorporated food security screening and referral and an environmental scan identifying implementation strategies and methods for screening and referral.

The National Governors Association (NGA) Center for Best Practices Health Division released a new guide, “The Future of Medicaid Transformation: A Practical Guide for States.” The guide provides a framework for how states can design statewide Medicaid transformation proposals, and is based on the NGA policy academy attended by three states that initially pursued Section 1115 waivers (Alabama, Nevada, and Washington).

This technical assistance document aims to help State and local government offices understand how Title II of the Americans with Disabilities Act (ADA) applies to their programs, activities, and services. The illustrated guide outlines general nondiscrimination requirements, like provisions regarding communication with people with disabilities or service animals. The guide also tells how the 2010 ADA Standards for Accessible Design apply to the built environment.

This is the official White House Conference on Aging (WHCOA) Toolkit that organizations and individuals can use to host their own events and submit to the official 2015 White House Conference. This report is a write-up of priorities and recommendations developed from a pre-White House Conference on Aging (WHCOA) event hosted by the Washington Association of AAAs (W4A) and the Washington State Council on Aging in October 2014. All four WHCOA issue areas were discussed with a group of about 125 people made up mostly by State Council on Aging members, AAA Advisory Council members and staff invited from all of the AAAs.

This tool promotes consumer-focused MLTSS by making it easier to identify weaknesses and promote better practices. The tool combines a checklist for assessing MLTSS with examples of practices, policies or contract language that implement the criteria.

This toolkit helps health plans and provider groups effectively engage, learn from and strategize with their diverse members and communities about the best ways to deliver person-centered care and support.
