Summary
The Advancing Chronic Care with Effective, Scalable Solutions (ACCESS) Model offers technology-supported care options for individuals with Original Medicare who have certain chronic conditions, including high blood pressure, diabetes, chronic musculoskeletal pain, and depression. Starting in Spring 2027, CMS will expand the model to include options for individuals with heart failure, chronic obstructive pulmonary disease (COPD), substance use disorders, and nicotine dependence.
The voluntary model is designed to integrate within the broader health care system, enabling healthcare professionals, such as those in primary care, to refer people with Original Medicare to ACCESS-participating organizations that will work with their care team. Currently, 160 organizations participate in the ACCESS Model, and can provide virtual care, health coaching, remote monitoring, and connected devices and wearables between regular doctor visits. The ACCESS Model uses a payment approach that emphasizes achieving specified health outcomes.
