WorkSource Wisconsin Employer Disability Resource Center

Wisconsin developed the WorkSource website and phone and email hotline to answer employers’ questions about recruiting and employing people with disabilities. Here you can find a focus group and needs assessment report used in developing the website. There are also a WorkSource brochure, a list of questions that can be answered within 24 hours, and a link to the site.

Federal Disability Programs: Coordination Could Facilitate Better Data Collection to Assess the Status of People with Disabilities

While some health and economic data on people with disabilities are currently available, these data have limited use in providing a comprehensive assessment of how these individuals are faring. Additionally, the lack of more useful data limits the federal government’s ability to determine how well federal programs are serving individuals with disabilities. Aging and Disability Resource Center (ADRC) Readiness Assessment

This assessment is designed to assist organizations in evaluating their readiness to perform key functions of ADRCs. The online version provides users with immediate feedback and links to resources to help build needed capacity. Quality Assurance and Improvement: DataMart Software Requirement Specifications and Reports

The goal of the QAI Datamart is to develop a consumer-based reporting and data analysis database using the Home and Community-Based Services Quality Framework. The QAI DataMart will support data analysis and decision support for quality assurance and improvement. The purpose of this Software Requirements Specification (SRS) is to document the customer’s requirements for the applications being developed or enhanced. The reports and queries available from the DataMart are also included.

Checklist for Enhancing the Participation and Input of People with Disabilities

In many ways, consumers are the best experts on their disabling conditions, including their own needs for support and the effectiveness and quality of programs. As a result, obtaining consumer participation and input at all stages of program design, implementation, evaluation and quality assurance is important. Information & Referral Database Inclusion/Exclusion Criteria

The Florida Department of Elder Affairs has set forth inclusion and exclusion guidelines for the integrated statewide information and referral database. This database will serve as a single point of access for information, counseling, referrals, assessment and eligibility functions, which will maximize consumer choice. The database includes entities that serve both Florida’s elders and their families, and adults suffering from severe and persistent mental illness.

Shared Master Index/Common Access Protocol - Minnesota

Minnesota's Shared Master Index will allow for the efficient exchange of information. By assigning each human services client an individual identification number that is used across multiple service delivery systems, the Shared Master Index (SMI) will allow caseworkers to view and assess the range of services that individual clients and families are receiving, and allow program managers and policy-makers to improve coordination of services and client outcomes.
