This guide was created as a resource to help you learn more about communication boards and how/when to use them. The boards in this kit were designed for a specific communication interaction, an intake interview to consider transition of an individual out of a nursing home. This handout provides a brief summary of the communication boards provided and some background information for you as you use the boards.

The Medicare/Medicaid Integration Program aims to end the fragmentation of financing, case management, and service delivery that currently exsists between the two programs. A report focusing on integrating information is provided, in addition to a link that provides several technical assistance reports.

Wisconsin’s Functional Screen system is a web-based application used to collect information about an individual’s functional status, health, and need for assistance for various programs that serve the frail elderly and people with developmental or physical disabilities. The website includes numerous resources regarding both an adult and children's version.

The report also describes how the state has been particularly innovative in using information technology to improve the quality of community supports and participants’ access to services.
