Nursing Facility Transition & Diversion - Module 2: Independent Living and Person-Centered Planning

It discusses the history and philosophy of independent living programs and definitions of person-centered planning. Michigan developed a six-part training on transition and diversion. Module 1 is an overview of transition and diversion in the long-term care system. After a brief introduction to nursing facility transition programs, the presentation dispels some myths about the outcomes of transition and diversion programs. In addition, Module 1 discusses different aspects of nursing home transition and the legislation and lawsuits that have affected this process.

Legisbrief is a briefing papers that offers important issues of the day. This volume provides an overview of new ways to help residents of nursing homes who want to return to their own homes but still receive services. The federal New Freedom Initiative awards grants to help states find ways to enable nursing home residents to use community-based care instead.

Money Follows the Person Initiatives of the Systems Change Grantees

This report highlights the work of nine CMS Money Follows the Persons Grantees, with a focus on Texas and Wisconsin. The report describes the initiatives, and discusses policy and design factors states should consider when developing MFP programs, including developing legislation and budget mechanisms for making transfers of funds, assuring availability of services and housing, identifying potential consumers for transition, developing NFT infrastructure, and monitoring and quality assurance.

The New Jersey Association of Centers for Independent Living website provides resources that can help users transition successfully. Centers for Independent Living (CILs) are consumer-driven organizations, owned and operated by individuals with disabilities. New Jersey has a network of 12 CILs that provide the core services of Information and Referral, Peer Support, Independent Living Skills Training, and Advocacy. Money Follows the Person Act - New Mexico

The New Mexico legislature has passed the \"Money Follows the Person Act\" (MFP) allowing people with disabilities of all ages to choose community living. The passage of MFP dove-tails with federal legislation signed into law by President Bush in the Omnibus Budget bill. The New Mexico legislation will allow a person in a nursing facility to choose community living, and have the money that has supported the person in the facility to follow the person to the community.

The New Hampshire Home Care Connections Project: Results of the Needs Survey at Baseline Summer 2005

This baseline survey has the reviews of professionals working with those at risk of losing their subsidized housing and entering a nursing facility due to emotional, cognitive, or behavioral issues. The survey was also used to better understand and identify risk factors, and import services, associated with nursing home placement, its avoidance, and training needs.

Locating Hurricane Survivors Placed in Facilities

Dr. Margaret Giannini, Director of the US Department of Health and Human Services (HHS) Office on Disability needs our help to locate hurricane survivors who were placed in nursing homes and other restrictive facilities. HHS is enlisting our help so they can work with HUD to get these folks back out of the nursing homes and into temporary housing in the community. The attached memo details ways you can send specific information about survivors.

Delaware Passport to Independence: Presentation, Assessment and Survey Forms

The Delaware Passport to Independence goals are to: identify residents of nursing homes who may want to move to a new home; interview and assess residents for possible community living; explore if it’s feasible for some residents to transition; create an Individualized Service Plan for those choosing to be considered. They have created a number of tools during the project development including an education outreach presentation, assessment form and survey tool.
