Request for proposals

This RFP is for the performance of services to ALTSD for New Mexico's self-directed waiver program, Mi Via.

This Request for Proposals was developed for the Consumer-Directed Care Plus Program. The F/E A RFP includes sections on minimum programmatic requirements, financial specifications, and contract provisions.

Community Living Program and Aging & Disability Resource Center grantees, and Veteran Directed Home and Community Based Services programs, evaluators, researchers, and other stakeholders are welcome to answer this call for proposals. Attendance is invitation only, but interested subject matter expert are encouraged to submit sessions and/or recommend a consumer to attend. Deadline is Dec. 15, 2009, “Changing Systems, Changing Lives” will be Feb. 22-24, 2010 in Alexandria, VA.

Ohio seeks a contractor to coordinate the NCI survey and related interviews involving 400 subjects. The purpose of studying the indicators is to understand service outcomes, compare Ohio to other states, align the state with the CMS HCBS Services Quality Framework, and inform strategic planning. The RFP, a schedule for proposals, blank survey forms, and a “bidder’s library” are given. Proposals are due June 5, 2009.

The MD Department of Aging envisions the Maryland Access Point (MAP) website as a place where older adults and people with disabilities can find information about services and resources in the state. MAP supports the work of local Aging and Disability Resource Centers (ADRCs). Use this closed RFP as an example if your program is searching for assistance in website development and maintenance.

Model communities “fully engage and use their social, economic and cultural resources to improve the quality of life for all of their citizens.” Wisconsin used this RFP to award MIG funds to communities for use in developing and implementing action plans to create a model community fitting that definition.

The MN Board on Aging, the agency with primary responsibility for MinnesotaHelp.info, issued this RFP with the goal of contracting an organization to maintain the database underpinning the website. The site is an online directory of services helping people in Minnesota find human services, information and referral, financial assistance, and other forms of help. Proposals must be submitted by Friday, May 30, 2008.

This RFP sought a contractor to implement a widely-used system performance measurement and monitoring program for state agencies serving individuals with disabilities. The resource includes a summary pamphlet and a detailed request. Use this example in developing a RFP.

CMS has available approximately $8 million in funding to continue to support States' efforts to address complex issues in long-term care reform. In addition, CMS was also awarded $5 million for Aging and Disability Resource Center /Area Agencies on Aging grants. Applications are due no later than July 17, 2008. Please see the full solicitation for details. Also included for background is a list of FY2007 grant recipients focused on person-centered planning and state profile tools.

CMS was soliciting proposals to develop the infrastructure to support competitive employment opportunities for people with disabilities. Funding is intended to facilitate enhancements to state Medicaid programs and services, to promote linkages between Medicaid and other employment-related service agencies, and to develop a comprehensive system of employment supports. The demonstration period runs Jan–Dec 2008. This prior solicitation acts as a resource for terms, conditions and guidance.
