2007 Community Alternatives to Psychiatric Residential Treatment Facilities (PRTF) Demonstration Grant Announcement

CMS announced $218 million in demonstration grants awarded up to 10 States to help provide community alternatives to psychiatric residential treatment facilities for children. These demonstration grants enacted by the Deficit Reduction Act, available over a five year period, will also assist States in adopting strategic approaches for improving quality as they work to improve each child's functional level in the community. Applicants' teleconference Sept. 19 & proposals are due Oct. 18, 2006.

Establishing Reliability and Validity in Ohio’s Individual and Family Survey: Request for Proposal

The Ohio Department of Mental Retardation and Developmental Disabilities (ODMR/DD) is issuing this Request for Proposals (RFP) to engage a contractor in a partnership to review the current Individual and Family (Draft) Survey items, recommend changes as needed to complete the development of the survey, establish the reliability and validity of it’s measures, ensure integration with the Core Indicators identified within Ohio’s Quality Framework, and develop a final draft of the survey.

2007 Money Follows the Person (MFP) Grant Announcement

Enacted by the Deficit Reduction Act, the MFP Rebalancing Demonstration is a part of a comprehensive, coordinated strategy to assist States in making widespread changes to their long-term care support systems. With the history and strength of the Real Choice Systems Change grants as a foundation, this initiative will assist in reducing reliance on institutional care while developing community-based opportunities. Request for Proposal and Certification Rules for Consumer-Directed Personal Care: New Hampshire

Granite State Independent Living (GSIL), working collaboratively with the Department of Health and Human Services (DHHS), solicited proposals from organizations interested in becoming consumer-directed personal care services providers (PSCP). Although this RFP is a few years old, it serves as an example of how to seek out providers. Moreover CHAPTER He-P 600 sets forth the procedures and requirements for operating as an other qualified agency.

Notice of Procedures for Designation of Consortia as a Participating Jurisdiction for the HOME Program

HUD CPD Notice 06-04 provides guidance on the HOME Investment Partnerships Program. The Notice updates CPD Notice 04-05; it is applicable to a) Units of General Local Government (UGLGs) that wish to form a consortium to participate in the HOME program, b) existing consortia that are already qualified to participate in the HOME program and want to add one or more UGLG members, c) existing consortia that must execute a new consortium agreement because their consortium qualification period ended.

Real Choice Systems Change (RCSC) Grants Solicitation FY 2006

With this funding for Systems Transformation, CMS will be awarding several states and non-profit agencies with small supplemental grants. States and other eligible organizations, in partnership with their disability and aging communities, may submit proposals to design and construct systems infrastructure that will result in effective and enduring improvements in community long-term support systems.

Quality Management and Performance Measure Development RFPs: Ohio

Ohio is seeking a single organization/individual to assist with development of two ODMR/DD’s Quality Assurance and Quality Improvement projects. The first RFP is for Quality Management strategy, the program and data analysis leading to implementation of the quality information management system and the use of a data for decision-making in a quality cycle. Evaluation of Ohio\'s PASSPORT Program: Request for Proposal

The Ohio Department of Aging (ODA) is seeking proposals from parties interested in organizing and executing an independent evaluation of PASSPORT (Preadmission Screening System Providing Options and Resources Today), the state's home and community-based services Medicaid waiver program. The program provides in-home alternatives to nursing home care for low-income seniors. The RFP and supporting material serve as an example of the products necessary to produce during a proposal process.

Request for Proposals: Measuring Quality through Individual Experience Surveys

The Texas Department of Aging and Disability Services (DADS) issued a RFP for an independent contractor to conduct consumer surveys by soliciting consumer feedback. DADS provides a comprehensive array of aging and disability services, supports, and opportunities that are easily accessed in local communities. The project will survey people recieving services through face to face interviews and mailings. This document serves as a guide for other grantees looking to prepare an RFP.
