Integrated Service Coordination Models for Housing Providers

ACL’s Housing and Services Resource Center will hold a webinar to highlight how housing providers can use service coordination to best support older adults and people with disabilities living in subsidized housing.

These populations often have unmet needs for supportive services. Service coordination between housing providers and disability and aging organizations can promote health, wellness, housing stability, and community partnerships.

Participants will learn from partners at the National Church Residences, which has successfully scaled service coordination across subsidized housing properties and leveraged community partners to deliver on-site programs.
