Capturing Data to Measure the Impact of CDSME on Social Connection

ACL’s Commit to Connect initiative will hold a webinar on the value of outcome evaluation of social connection within chronic disease self-management education (CDSME) programs.

In 2025, Commit to Connect launched the Understanding the Impact of Social Isolation and Loneliness Programs Community of Practice (CoP) to support six community care hubs in conducting retrospective evaluations of social connection outcomes in CDSME programs. 

This webinar will highlight CoP findings and best practices on measurement and evaluation from participating organizations.
