Webinar: Supporting Patients With Chronic Illness Across Care Settings

The ACL No Wrong Door (NWD) Resource Center is hosting a webinar exploring how community-based organizations (CBOs) can support individuals with chronic conditions during key care transitions.

Attendees will learn about:

• Critical intervention points where community-based organizations can support individuals during hospital discharge planning, transition back to the home, and post-discharge follow-up
• Long-term supports that help individuals remain in their communities and avoid hospital readmissions
