New Resource and Webinar on Community Health Integration and Principal Illness Navigation Medicare Codes

Event Description:

The Partnership to Align Social Care (Partnership) recently released a resource titled "Understanding the Medicare Physician Fee Schedule Billing Codes for Community Health Integration (CHI), Principal Illness Navigation (PIN), and Principal Illness Navigation – Peer Support (PIN-PS) Services." This resource provides an overview of the core concepts required to implement community health integration (CHI) and principal illness navigation (PIN) services. The CHI and PIN service codes provide a direct pathway for reimbursement for work performed by auxiliary personnel, including community health workers, to the Medicare provider. The Medicare Physician Fee Schedule allows providers to contract with community-based organizations, such as Area Agencies on Aging (AAA) and Centers for Independent Living (CIL), to provide staffing to address identified health-related social needs (HSRNs).

The Partnership will also host an upcoming webinar to discuss this resource, providing an opportunity for questions and answers to inform planning for and implementation of CHI and PIN services.

This webinar will take place on Wednesday, April 3 at 12:00 p.m. ET.  For more information or questions, contact info@Partnership2asc.org.
