Home and Community-Based Services for Comprehensive Dementia Care

The National Dementia Care Collaborative (NDCC) will hold a webinar on practical ways to strengthen connections between home and community-based services (HCBS) providers and clinical care providers.

Speakers will share how their organization turns person-centered care plans into tailored support for people living with dementia and their family caregivers. The team will share results from its work with GUIDE beneficiaries and offer tools to help increase HCBS use for GUIDE participants, as well as dementia care providers, primary care clinicians, or respite partners.
