Event Description:

The Aging and Disability Business Institute is hosting a webinar with Partners in Care Foundation. During this webinar, Partners in Care will discuss how their partnership with a local medical provider helped them develop a model of transitional care management and chronic care management focused on addressing the Social Determinants of Health. The webinar will provide an overview of the CMS policies which allow CBOs to contract with medical providers and discuss specifics such as billing codes and reimbursement rates. Participants will also learn about the planning process for deploying the integrated model of care.
